Sub Acute Rehabilitation Program
At Laurel Bay, our short term Sub Acute Program is designed to meet the needs of residents requiring post-acute hospital rehabilitation services. Our goal is to ensure that each of our residents achieves and maintains an optimal level of functioning and independence through quality rehabilitation and educational programming.

Our focus is to design an individual program to meet both realistic goals and resident expectations. This is achieved by including both the resident and family in the treatment planning process. Our clinical team will ensure a smooth transition during this process and our Licensed Clinical Social Worker will assist in making appropriate discharge arrangements.

Long-Term Care
We at Laurel Bay are long-standing members of the community and our residents are part of our family. Our Certified Therapeutic Recreation Specialist has designed a program to meet our residents needs and interests. Additionally, our residents receive the benefits of all our specialty services, with the community and compassion we are known for, in a long-term family setting.
